เรื่องราวในเกม Fallout: New Vegas จะเกิดในสามเมืองใหญ่คือ New California Republic กับ Caesar’s Legion และ New Vegas เหตุการณ์ในเกมจะเป็นเวลา 3 ปีหลังเหตุการณ์ในเกม Fallout 3 และเป็นเวลา 40 ปีหลังจากเหตุการณ์ในเกม Fallout 2 ในส่วนของ New Vegas ไม่ได้เสียหายจากระเบิดนิวเคลียร์มากนัก ยังมีบางส่วนที่ยังคงสมบูรณ์และมีพืชพันธุ์แบบปกติที่ขึ้นอยู่ตามทะเลทราย
       
ในภาคนี้เราจะได้เล่นเป็นคนส่งข่าวที่ขนพัสดุเดินทางข้ามทะเลทรายและ ก็ตื่นขึ้นมาจากหลุมศพ ถูกขุดออกมาจากหลุมโดยเจ้าหุ่นยนต์จากนั้นก็ได้รับการช่วยเหลือจากหมอคน หนึ่ง หลังจากฟื้นตัวได้เราก็จะได้รับการทดสอบผ่านระบบ SPECIAL (Special: Strength, Perception, Endurance, Charisma, Agillity, Luck) แล้วคุณหมอก็มอบ Pipboy ซึ่งเป็นคอมพิวเตอร์ขนาดเล็กส่วนบุคคลและใช้ติดไว้ที่แขน จากนั้นก็ออกสืบหาว่าใครที่เป็นคนจะสังหารเราในตอนแรก
